Treatment Disclaimers

Dental Implants

Not everyone is a candidate for dental implants. Candidacy depends on numerous factors including bone density, overall health, oral hygiene habits, and other individual circumstances. There is no way to accurately predict healing capabilities for any individual patient. No guarantee, warranty, or assurance has been or can be given regarding implant treatment outcomes.

Dental implant placement and restoration may be performed by multiple treating providers. The precise roles of each provider in your treatment will be discussed during your consultation.

IV Sedation

IV sedation is an elective procedure that carries inherent risks including nausea, vomiting, allergic reactions, breathing complications, and in rare cases, serious medical complications. Patients receiving IV sedation must arrange transportation, have an adult escort, and follow all pre-operative and post-operative instructions. Detailed informed consent will be obtained in person before any sedation procedure.

Bone and Gum Grafting

Grafting procedures involve additional surgical sites and carry risks including graft failure, infection, prolonged healing, and the need for additional procedures. Different graft materials carry different risk profiles, which will be discussed during your consultation.

Full Arch Restorations

Full arch restoration procedures require ongoing maintenance, professional care, and excellent oral hygiene. Results are not permanent and may require repair, adjustment, or replacement over time. Success depends significantly on patient compliance with care instructions.

Appointment Scheduling

Online Scheduling

Appointment requests submitted through our website are requests only and are not confirmed appointments until you receive confirmation from our office. We will contact you to confirm your appointment during our regular business hours.

Cancellation Policy

We request at least 48 hours' notice for appointment cancellations or rescheduling. This allows us to offer the appointment time to other patients who need care. Missed appointments or late cancellations may result in a fee. Our detailed cancellation policy will be provided when you schedule your appointment.

Insurance and Payment

Payment is expected at the time of service. We will file insurance claims as a courtesy; however, you are responsible for payment regardless of insurance coverage. Insurance estimates provided are estimates only and do not guarantee coverage. We encourage you to verify benefits directly with your insurance provider. We do not process payments through this website; all payments are handled at our office.

HIPAA Acknowledgment

As a HIPAA-covered entity, our Practice maintains the privacy and security of your Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act and applicable state law. Note: This arbitration agreement does not apply to disputes regarding dental treatment or malpractice claims, which are governed by Nevada medical malpractice law and may be subject to different procedures.

Telehealth and Teledentistry

To the extent our Practice offers teledentistry services, such services are subject to these Terms, our Privacy Policy, and the following additional terms:

As required by Nevada law (NRS 631.34583), before receiving teledentistry services, you will be informed of:

  • The types of services that may be provided through teledentistry and their limitations
  • Information about each licensee providing teledentistry services
  • Precautions to be taken in the event of technological failure or a medical emergency

Teledentistry is not appropriate for all situations. Emergency conditions, certain examinations, and most dental procedures require in-person evaluation and treatment. Teledentistry consultations do not replace the need for regular in-person dental care.

By using any teledentistry services, you acknowledge that you have received our Notice of Privacy Practices and consent to teledentistry services after receiving the required disclosures.

Severability

If any provision of these Terms is found to be invalid, illegal, or unenforceable by a court of competent jurisdiction, such finding shall not affect the validity of the remaining provisions, which shall continue in full force and effect. The invalid or unenforceable provision shall be modified to the minimum extent necessary to make it valid and enforceable while preserving its original intent.
